Massive left hemothorax following diaphragmatic and splenic rupture with visceral herniation is quite an uncommon life-threatening condition usually associated blunt thoracoabdominal trauma. Mortality generally coexistent vascular injuries that could be rapidly fatal. Timely, proper diagnosis mandatory as survival depends on prompt treatment. We describe a case of massive secondary to injury rupture, gastric, greater omentum into the thoracic cavity in 32 years old male car driver after sustaining road traffic accident presented shortness breath 4 h’ duration. He also had zone 3 retroperitoneal hematoma acetabular fracture. was treated surgically discharged home improved.
